Dear Sir,
I make this letter to make complaint regarding the products that I purchased recently.  I bought a numerator with serial number 1236 and  hanging folder with serial number 7890 and product number no. 567 and no. 340 from one of your outlet in Ahmad Yani Street , no. 57 , Senayan.
The product did not work properly. For a hanging folder it cannot be closed properly and for the numerator the number is missing. I have lodged a complaint on your customer service center , but there is no response or action has been taken till now.
I want you to take action to resolve this problem as early as possible. I attach a copy of the invoice and product photo.

Surat ini ditunjukkan kepada Bapak Joko Anwar sebagai manajer customer service , memberitahukan bahwa barang yang dibeli oleh PT XYZ berupa numerator dan hanging folder disalah satu otletnya di jalan ahmad yani senayan tidak dapat digunakan dengan semestinya , PT XYZ sendiri sudah komplain tetapi tidak ada respon maupun tindakan dini, maka dari itu PT XYZ mengirimkan surat ini ke bapak Joko

This letter is for Mr. Joko Anwar as Customer Service Manager . For items order by PT XYZ (numerator and hanging folder) from one outlet in Ahmad Yani street,no 57, Senayan cannot be used correctly. PT. XYZ has reported it but there are no responses or action.
